Developing A Picture/Profile Of Your Child
A transition team should hold the parent’s assessment and observation of their child in high regard. After all you know best how your child is likely to respond to change, act in a new environment and to new adults and peers surrounding him/her. Creating a profile of your child helps your transition or child study team better understand your child’s needs and current status. It also allows the conversation to stay focused on the child by having visual reminders of her front and center of the team throughout the discussions.
We offer two options for families to complete. Some families choose to complete this profile exercise send it to the Child Study Team contact person before the evaluation or Individualized Education Program (IEP) meeting.
OPTION 1
You may choose to complete this description of your child and send it to the Child Study Team Contact Person before the Evaluation or Individualized Education Program meeting to provide a picture of your child which may be used in establishing your child's present levels of education performance.
